Thank you for taking the time to fill out our survey. Please note that the survey
is completely anonymous. No personally identifiable information will be associated
with your ratings. If you have any questions or concerns about the survey, please
contact us at staff@symlog.com.
At the start of the process, you will be given an authorization number. Please write
this number down. If you get interrupted and need to resume your ratings, you may
use this number to log back into the system and resume rating where you left off.
Also, if you have any technical problems with the rating system, we will need this
number to be able to help troubleshoot your issue.
You will be asked to answer a series of 4 questions, each with a different focus.
Please read each focus carefully before answering the question. Some of the items
may seem conflicting or vague, so if any part of the item applies, it should be
used as a guide and rated accordingly. It should take you no more than 3-5 minutes
per question. Please try to answer all of the questions in one sitting.
We will be running this survey for several weeks, so please encourage your adult
friends and colleagues to make ratings during that time. Once we have finished collecting
ratings and analyzed the data, we will publish the results here as well as in our
quarterly newsletter.
